Jan M. Werth, age 81, passed away on Wednesday, November 15, 2017 at HaysMed in Hays, KS.

A graveside service will be held on Monday, November 20, 2017 at 3:00 PM at the Mt. Allen Cemetery in Hays, KS.

A memorial visitation will be from 2-3:00 PM at the Brock’s-Keithley Funeral Chapel & Crematory, 2509 Vine St., Hays, KS 67601.
